Describe it well in the body of the question. Add the rest in Details.
If you post a picture, make sure it's clear, not too small (resolution is poor, so if we have to blow it up it might be unreadable), and right side up.
Show the work you've already done, so we know you've tried, and aren't just asking us to do all the work for you.
Define any non-standard notation. Don't presume we know the conventions your book or prof use.
Proofread before posting. Make sure the question is clear and complete. Add an update if anything is wrong.
Really, really, hard questions DO get answered here. There are math PhD's here that love advanced questions.
If you think it may have been asked already, do a search for it first. If you think the internet has an answer, look for it there first.
If it's just arithmetic or units conversion, use a calculator or google. Don't waste our time.
Crap like "what two numbers add to this and multiply to that" are soooo boring. Nonsense like "what's 10+9" or "what's 1+1" or the same question over and over again will get you reported as wasting resources.
Only post it ONCE. If you accidentally post it multiple times, delete the extras.
Be sure it's posted in the proper category. Y!A assigns one. Change it if it's not the most accurate (it's under Edit).
If it's a process or technique you don't understand, asked for a detailed explanation.
Don't use the lame excuse "my teacher sucks" or "I missed that class." You have other resources. Like a textbook or Wikipedia. Use them first.
That's what we look for, and what annoys us.

Solution: Ask easier questions.
If you ask really difficult questions, then users will either not know how to solve it or not care enough to expend the effort to solve it.
